Treament of Central Vein Stenosis with Percutaneous Transluminal Angioplasty

Abstract

|
Percutaneous transluminal angioplasty (PTA) has played an important role in the manageent of malfunctioning fistula. Stenotic lesions, most commonly occurring in the venous limbs of such fistulas, are the most common anatomic cause of malfuction.
Central veinstenosis has emerged as serious new medical complication of hemodialysis catheter insertion. These central vein lesions are not amenable to simple surgical revision or thrombecomy because of their intra thoracic location. Previous
reports
ahave documented the excellent response of subtotal central vein lesions to PTA, however, there was no report in Korea. We performed PTA succesfully in 1 patient presenting with near-total obstruction of the subclavian vein after subclavian
catheterization and arterio-venous fistula at the same side. The pressure difference across the stenotic site was 30 mmHg, which decreased to 0 mmHg after PTA. Arm swelling and collateral circulation also decreased remarkably after PTA.
